Inspect once a year and you have one observation of a system that changes daily. The gap between what an inspection tells you and what is true is not a failure of the inspector. It is a property of sampling.

Failure is not an event

A fire door does not fail at a moment you can schedule around. Hardware drifts, closers weaken, and a door held open on a warm afternoon is non-compliant for exactly as long as nobody walks past it.
